New Patient
FormsBefore your first visit, we’ll ask you to complete a New Patient Form. We know these forms can feel a bit lengthy, but every question serves an important purpose. The information you provide helps us understand your health history, any current concerns, and factors that may be relevant to your assessment.
As chiropractic care can involve manual techniques and physical assessments, it is essential that we identify any contraindications - health conditions or circumstances that may mean certain assessment or management approaches are not appropriate for you. Your responses allow us to make safe and informed decisions during your visit.
We encourage you to answer the questions as accurately and thoroughly as possible. If you’re unsure about anything, our team is available to assist and clarify what is needed. This helps us ensure your appointment is conducted in a way that prioritises your comfort and safety.
Pregnancy Peadiatrics
&When caring for pregnant patients or children, the overall process is similar to what we do for all new patients, but the details look a little different. Both groups have unique physiological needs, so their new patient form and assessments are tailored to ensure we gather the right information and provide care that is appropriate and safe.
For paediatric visits, we recognise that children are not simply small adults! Their bodies develop in stages, which means their assessment and management needs are different. Our paediatric New Patient Form therefore asks about birth history and developmental milestones, as well as their health history. This helps us understand important factors that may influence how we approach their appointment.
Pregnant patients also have different needs, and their care reflects the changes their bodies undergo throughout pregnancy. As ligaments become more relaxed and body mechanics shift, our techniques are modified to ensure they remain comfortable and safe for your stage of pregnancy. We use pregnancy support pillows from the second trimester onward to allow you to lie face-down comfortably during your appointment; however, we can also accommodate for a different lying position if you prefer. These supports can also be helpful post-partum, especially if you are experiencing tender breasts or general discomfort after birth.
